CHI St. Francis Health patient admission process is smooth and efficient. Registration may take approximately 10-15 minutes.

Items you will need to bring with you on day of admission:

All active insurance cards
Picture ID
Any co-payment/deductible that can be made. (We accept MC, Visa and Discover)

Pre-registration

You may receive a pre-registration call from us in advance of your day of admission. We encourage all patients to complete this pre-registration step to minimize wait times.

Items you will need for this pre-registration process:

All active insurance cards
Social Security numbers for patient and insurance subscriber
Physician Order - information about your doctors, including names and addresses
Emergency contact information
